I followed the suggestion to do a system restart on the TV and it worked. Sometimes restarting my Xfinity cable box has helped but not always. This time I went to the Home icon on my Roku remote then: SETTINGS -> SYSTEM -> POWER -> SYSTEM RESTART. When opening the Amazon Prime App, it took a few seconds but the icons came back.

Re: Closed Captioning w/Amazon. It is Amazon's app. They adapt it for each platform they wish to support and supply this customized version to that platform, in this case to Roku. Roku will pass your key presses on to the Prime app but what the app can do with them depends on what Amazon has programmed into the version they supplied to Roku.

Sorry for the delay in answering. Our Amazon Prime subscription lapsed and we waited a little while before starting it back up again. As a reminder, this issue happens on the Prime Video channel only. All of our other channels work correctly. Also, this issue happens on two different Roku devices (Roku Premiere and Roku Express).

I just launched the Amazon Prime channel after PKinVA's post, and it's totally different looking, meaning the channel had a major update, so it may be glitchy for a bit till Amazon resolves all the glitches. I repeated the steps users had and didn't experience the crash, so my Roku Ultra device doesn't seem to be affected currently.
